QA Technologies is seeking a highly skilled Website developer with solid database skills to develop a Wellness Challenge Website for the State of Nevada. Position is located in Las Vegas and will have a flexible onsite/offsite arrangement.

The Wellness Challenge Website is being developed through the State of Nevada Department of Education, Office of Child Nutrition and School Health. This activity and intake will be matched with the goals of the dietary guidelines and reported to the participant. On a weekly basis an email would be generated to the participant indicating their progress on attaining one of five levels of achievement. If no input for the week, the program would generate a generic message about missing their information.

The five levels would be established on an individual**s age, gender, height, weight, and fitness level. Based on the individual**s goal and personal information, the program would recommend the level to begin their journey to achieving a healthy lifestyle. The five levels would begin with an entry level building to the last level in which the participant has met or exceeded the Dietary Guidelines for their age and body mass index (BMI).

Once the individual has reached their desired goal level, the program would automatically generate a certificate of achievement.

Data collected on the site would be available for other State agencies. All FERPA and HIPPA rules and regulations must be adhered to.

The Nevada Department of Education, Office of Child Nutrition and School health will be responsible for the maintenance of the Website.

The consultant must develop the Web-based software to gather personal information, provide resources, and a methodology for collecting progress for an individual participating in the Wellness Challenge.
